DATA PRIVACY NOTICE AND CONSENT FORM

Philippine Foremost Milling Corporation (“PFMC” or “Company”) is committed to the protection and proper processing of your personal information in accordance with the Republic Act (R.A.) 10173), otherwise known as the “Data Privacy Act of 2012”. The Data Privacy Act of 2012 sets out principles in processing of personal and sensitive personal information and mandates the Company to inform you, as the Data Subject, that your personal data is being collected, the manner and the purposes for its collection, and the type of personal data being collected.

This Data Privacy Notice and Consent Form serves as the Company’s compliance to the Data Privacy Act, and by signing this form, you grant your free, voluntary and unconditional consent to the collection and processing of your Personal Data (as defined below) by the Company, authorized agent/s and/or representative/s as information controller, by whatever means in accordance with the “Data Privacy Act of 2012”, including its Implementing Rules and Regulations (IRR) as well as all other guidelines and issuances by the National Privacy Commission (NPC).

The Company processes, collects, uses and keeps your personal information, which include but are not limited, to the following:

·         Contact Number

Manner and Purpose of Collecting Personal Data

 

The Company collects and processes your personal data through this website. Any personal data collected shall be used only for the following purposes:

 

(i)             To enable the Company to comply with the mandate of existing laws and regulations of both national and local governments;

(ii)            To update and inform me of the products, services, promotions, trainings, and programs of the Company; and

(iii)           For such other legitimate purposes implied from or relevant to the foregoing.

The personal data collected shall be retained by the Company for as long as necessary for the fulfillment of any of the aforementioned purposes, and shall continue to be retained for a period of three (3) years notwithstanding the termination of the above-stated purposes, or for as long as necessary to comply with lawful order, subpoena, or requests from the court, tribunal, or other government office, agency, or department.

Sharing and Access to Information

The collected personal data may be accessed, used, and shared by the Company, its authorized employees, agents, contractors, service providers, and personal information processors (PIPs), solely for the purposes stated in this document, other legitimate purposes and shall be in accordance with applicable data privacy laws.

Data Protection

 

PFMC ensures the security and confidentiality of your personal information through the implementation of appropriate physical, technical, and organizational security measures, including but not limited to:

·         Restricting access of your personal data to authorized personnel only;

·         Secure storage of electronic files; and

·         Proper disposal or permanent deletion of information when no longer necessary, in accordance with the agreed retention period.

In the event of any Personal Data Breach, the Company shall promptly submit a report to the National Privacy Commission (NPC) in accordance with prescribed rules and regulations.

Rights of the Data Subject

 

You are hereby informed that, under the Data Privacy Act of 2012, your rights as a Data Subject include:

·         to be informed of how my personal data is collected, used, or processed;

·         to object to or withdraw consent for the processing of my personal information in case of changes or amendments to the purposes mentioned;

·         to request access to, correction, or updating of my personal information if it is inaccurate or incomplete; and

·         to suspend, withdraw or order the blocking, removal or destruction of my Personal Data from LFUG GOC’s information database system;

·         to file a complaint with the National Privacy Commission (NPC), if necessary.

By submitting the online form, I hereby certify that I have read and fully understood all the contents of this Data Privacy Notice and Consent Form in the language in which it is written. I voluntarily and without any form of coercion authorize and give my consent to the Company, its duly authorized agents, directors, officers, business unit heads, employees, contractors or personal information processors (PIPs) to collect, use, access, store, and in any reasonable manner process my personal data, whether personal or sensitive information, which I have voluntarily provided in this form for the purposes stated.
